New patient questions
Which forms do I need to complete?
New patients complete three items before treatment begins: the New Patient Forms, a Health History, and an acknowledgment of our Privacy Policy. All three are available from our office.
How do I submit my completed forms?
Complete the forms and either fax them to 410-874-2222 or bring them with you to your first appointment.
What if I can't complete the forms before my visit?
No problem — just arrive 10 minutes before your scheduled appointment and we'll help you complete everything at check-in.
What do I need to open the forms?
The forms are PDFs, which require Adobe Reader (nearly all computers already have it). If a form will not open, Adobe Reader is a free download from Adobe.
